My husband has a sinus infection and needs an antibiotic. He has had the infection for 5 days, has used antibiotics before, and has no allergies to medicine. He has lots of phlegm and mucus, pain in the sinus, and eats. He had a fever initially but doesn't anymore and is taking lots of over-the-counter medicine. He has yellow phlegm, a raspy and dry cough, no blood, no shortness of breath, sinus congestion, no headaches, blows his nose constantly with no relief, and has pain in his sinus and ears. He did have a sore throat the first day or two but not anymore. He is not allergic to any medication and has no other medical conditions.

I need a doctor's note for work. My symptoms include fever, headache, and yellow snot. The headache is a one-off, dull, and not localized to one area.
