I am a 35-year-old female with a history of one abnormal pap (LSIL) at age 22, which cleared on its own. My recent routine pap came back as HSIL, and I underwent a colposcopy with a biopsy that showed cells with changes raising suspicion for possible AIS. I am on Pristiq 50 mg daily and Visanne 2 mg daily, and have a family history of dedifferentiated endometrial carcinoma (mother). I am concerned about the possibility of invasive adenocarcinoma and the recommended treatment, including whether an aggressive LEEP or a cone biopsy is preferred, and the potential outcomes and risks involved.
