I’m 4 weeks postpartum after a c-section. My bleeding had stopped at 3 weeks, but for the past 2 days, I’ve been consistently bleeding with clots filling up a pad every hour. This is my first time experiencing this. How long can I wait before going to the ER?

I started using dilators about 3 weeks ago. It has been over 5 years since I've been intimate. I haven't dilated for a week now and today when I used the restroom and wiped, I noticed a bloody, almost mucus-like substance on the paper. The color isn't bright red, it's a darker shade of red. Should I be concerned? I also have a UTI and am using an ointment (nystatin & Triamcinolone acetonide) and a medication (SMZ-TMPDS sub for Bactrim DS). I've been using estradiol cream and Norethindrone Acetate and Ethinyl Estradiol tablets since March. I don't have any pain, but it feels like when I would get my period, as if I can feel my ovaries. Is this normal?
