She has completed a 10-day course of doxycycline, which reduced the swelling by 2/3, but the hard area and blood/pus expulsion persist. The affected area is about 3 inches long and 1 inch thick, and it is painful when touched. She was born with spina bifida and has been wheelchair-bound her whole life, with a history of a healed decubitus ulcer. We suspect she scratched herself, leading to infection due to her incontinence and prolonged exposure to urine. What should be done next?
