I am experiencing abdominal pain, neck pain, and intermittent nausea. I have an IUD and had a positive pregnancy test followed by seven negative tests. I haven't had a period in 7 years due to the IUD. What could be causing these symptoms and what should I do?

I'm having light pink spotting 13 days before my period. I use a spermicide and this is the first time it's happened. I had a sharp pain in my lower abdominal this morning, it felt like a period cramp. I've had 2 full-term pregnancies and last visited an OBGYN for a pap and STD screen 5 years ago. I took a pregnancy test about 2 months ago, but not since my last period. My last period came 2 days early and was 1 day shorter than usual. Could this be related to the spotting, and could rough sex also cause spotting?
