I am starting a 1099 position as a Psychiatric Mental Health Nurse Practitioner at a private practice. I will only receive 60% of the billed amount from patients/insurance. Can this be considered a loss/deduction for me on my taxes, and if a patient does not pay their bill after I have provided services, can this be written off as a loss?

I have filled out an SSA-1724-F4 form. Can I bring it to the Social Security office and slip it through a slot or something? Alternatively, can I FAX it? My husband passed away on 01/02/2021, and there was an issue with the December Social Security payment being taken back.
