I am disabled and took out a lump sum from my retirement. The amount was $4852, with $970.53 federal tax withheld. I am in Oregon and have no dependents. I received a tax refund of $1031 for 2019. I also had to pay $600 to Oregon. Is everything correct, and does Oregon tax the lump sum?

I have misplaced the Form SSA-1099 for 2020 for William H Hill and Binki M Hill. How can I obtain copies of these forms for our tax preparation?

I made qualified charitable contributions (QCCs) in 2020 from two IRAs for $20,000 and $30,000. The 1099-R forms showed both as ordinary income (line 1) and taxable income (line 2a). Is this correct, even though the sums do not count as income since they were QCCs? Turbo Tax will not let me e-file if I don't fill in line 2a.
