My 3-year-old Maltese Yorkie has been experiencing recurring diarrhea despite being on a bland diet and dewormer. The symptoms return when transitioning back to his regular dog food, which he has eaten for years. He has multiple bowel movements each morning, with the second being large and soft, and the third containing blood and mucus. Switching to a rice diet temporarily resolves the issue, but adding turkey back in causes symptoms to return. What could be causing this and what steps should be taken next?

My dog, Raider, a 10-month-old mix breed, ate a shrimp yesterday around 3pm and has been experiencing diarrhea since around 11pm. He has shown no signs of vomiting and is bright and alert with a good appetite. However, his stool has become more watery today and there was a bit of mucus and a tiny bit of blood observed. My 6-year-old, 65-pound Pit Bull ate a little less than a quarter of a vegan soap bar around 8pm. She ate a full meal immediately after and vomited lightly at 1am and the rest of her meal at 3am. She has no other symptoms and is sleeping soundly. Do I need to take her to the vet?
