I occasionally experience a tingling sensation in my legs accompanied by slight lightheadedness, usually when I get up from my desk or chair and walk briskly. I had a heart attack in 1994 and have had some muscular back pain recently. What could be causing these symptoms?

My mother has been experiencing severe, clenching liver pain radiating to her back, similar to gallbladder pain but without a gallbladder since 1980. The pain is unresolved by narcotics but somewhat relieved by Xanax. I have been suffering from CFS and adrenal exhaustion. I recently had a comprehensive parasitology test which showed low beneficial bacteria (Lactobacillus species), high levels of Enterobacter cloacae and Klebsiella oxytoca, and yeast (1+). I have been struggling with extreme fatigue for over a year now. My primary care physician found EBV and a prior case of mono, and a rheumotologist indicated RA based on anti-CCP tests. I took methotrexate for RA but it made me nauseous, and my ALT level is elevated. No one is addressing the fatigue, which is worsening, leading to frustration and depression. What tests should I undergo and what treatments might help?
