The document includes a clause stating I can't apply for SSD and Medicare, Medicaid for 30 months, which I find suspicious. I was injured at work in Florida on 03/10/2021 with injuries to my cervical spine, knees, lumbar, shoulder, and PTSD. I have moved to Maine and have been under treatment there until 06/02/2022. I am concerned about the contract's fairness and whether signing it could cause future problems.
