I had a visit to The Little Clinic in December for an abbreviated physical for cataract surgery and was told they do not file claims with insurance companies, even though they are in-network. I had to pay upfront and filed the claim myself, but the CPT codes used by The Little Clinic do not match the insurance company's requirements. I was told by law they need to file the claim but they refuse. The insurance company told me I am not responsible to pay for that service and I called to get reimbursement, but they refused. Where do I stand and can you help me?
