I have severe pain at my right side just under my rib cage that radiates to my back of shoulders. I have vomited one time about 2 hours ago. I have taken Gavascon but it does not help. I am a Lupus patient. What could be causing this pain and what should I do?

I had shoulder surgery in June for impingement, bursa removal, and shaving of the labrum and rotator cuff. The pain has returned in a different location, and I'm concerned about possibly tearing the rotator cuff again.
