My 48-year-old daughter, who is intellectually impaired, non-verbal, and in a wheelchair, has developed swelling of her left labia with hardness and expulsion of pus and blood from a small break in her skin. She has completed a 10-day course of doxycycline, which reduced the swelling by 2/3, but the hard area and blood/pus expulsion persist. The affected area is about 3 inches long and 1 inch thick, and it is painful when touched. She was born with spina bifida and has been wheelchair-bound her whole life, with a history of a healed decubitus ulcer. I have a rash around the bottom of my vagina and in the folds. It started about 4 days ago and is constant and uncomfortable, but not itchy. I have tried cortisone cream, diflucan, ciprofloxacin, and mupirocin, but the mupirocin cream causes severe burning. What could be causing the rash, and what should I do for treatment?
